The Occupational Therapist (OT) will provide restorative therapy to the client while using special techniques or providing adaptive devices to assist the client in gaining independence in activities of living in accordance with the plan of care.
The OT will also oversee the Occupational Therapy Assistants to assure appropriate implementation of the plan of care.
o Performs comprehensive subjective and objective admission and ongoing assessment of client functional status and activities of daily livingo Administers functional ability tests and competency as neededo Evaluate adaptive equipment needs and obtain equipment for cliento Formulate individualized therapy plan of care that incorporates analysis of assessment datao Provide therapy interventions aimed at achieving realistic outcomes within a specified period of timeo Modifies and updates the therapy plan of care to reflect progress toward outcomeso Participates in the development, review and updating of the care plano Consistently demonstrates competency with occupational therapy skills according to personal and legal scope of practiceo Evaluate the home environment and make appropriate recommendationso Instruct client, family and other team members in proper use of assistive equipment, prosthetic devices and adaptation deviceso Evaluates effectiveness of teaching and modifies accordinglyo Promotes client/family autonomyo Relays significant changes in status to physician and other members of the team in a time period consistent with client needo Maintains client records showing systematic assessment, planning, intervention and evaluationo Documents client charts, records and clinical notes legibly and according to documentation guidelines and standardso Supervises certified occupational therapy assistants according to organization policy and state regulations.

o Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
o Registered with the National Registration Examination of the American Occupational Therapy Association.
o Possess and maintains current CPR certificationo One (1) year experience as an Occupational Therapisto Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
